As part of our comprehensive gender-affirming care, Equitas Health partners with trusted LGBTQ+ advocate TransOhio to offer free legal clinics for the trans+ community on name and gender marker changes. At these monthly virtual clinics, a trained attorney will meet with you one-on-one to guide you through the process of changing your name and gender marker in the state of Ohio.

Step 1: Click here to complete and submit the intake paperwork.
Step 2: We will review your paperwork to make sure it is complete and contact you with available clinic appointment dates and times.
Step 3: Respond to us with the appointment date and time that work for you.
Step 4: We will confirm your appointment on a first come first serve basis. We will send you a Zoom link or phone number for your appointment shortly before the clinic.
If you experience issues accessing the intake paperwork, please email legalclinics@equitashealth.com.
We need your intake paperwork at least 48 hours in advance of the virtual legal clinic. If the number of clients exceeds the capacity of the volunteer attorneys, the legal clinic coordinator will work to accommodate you at a future legal clinic.
The average wait time for the virtual legal clinics is four to six weeks.
