Build your Whole Person Healthcare team with our physicians, nurses, pharmacists, mental health professionals, dentists, advocates, and financial counselors. They will make sure you get the care you need. Your Whole Person Healthcare team helps you take charge of your health and stay connected to care.

 

Primary Care means taking care of yourself even when you’re healthy.

Routine physicals, screenings, and vaccinations help keep you healthy. You and your primary care provider will talk about your current health risks. You will work together to lower those risks with choices that work for you.

Health screenings, like blood pressure and cholesterol checks, help you and your provider find health issues before they get serious. This means you can get treatment at an early stage.

Our Primary Care services include:

Routine physicals and regular health screenings, like blood pressure, glucose/sugar, and cholesterol checks, help you and your provider:

  • Keep an eye on your health risks.
  • Treat health issues early – before they get serious.

Our diagnostic services make sure you get the treatment you need.

Services include:

  • Cholesterol & glucose (sugar) checks
  • Testing and treatment for sexually transmitted infections
  • Testing and treatment for hepatitis B & C
  • Onsite lab draws, such as blood & urine tests

Our pharmacies offer many vaccinations – such as the COVID-19 vaccine – to keep you and your family healthy. No appointment needed.

Services include:

  • Family planning
  • Yearly exams
  • Cervical exams & pap smears
  • Birth control
  • Emergency contraception (the “morning after” pill)

Work with your care team to treat and control chronic health issues with medication and lifestyle changes. Yes, that is code for diet and exercise!

Your provider will help you get an appointment with a trusted provider or specialist when you need to get care somewhere else.

When you are ready to quit, we are ready to help!

Primary Care also means getting the care you need when you’re sick.

Getting sick is inconvenient. Getting better doesn’t have to be. That’s why we’re here when you need us most.

Trips to emergency rooms or urgent cares can cost you more money and time. Call Equitas to see if a same-day sick visit is a better choice for you.

Starting at 7:30 AM, call (833) 378-4827 and ask for a same-day sick visit.

24 hours a day, 7 days a week.

Speak to a live nurse when you call your local Equitas Health Medical Center outside of normal office hours.

They will let you know if you should go to the ER, an urgent care, or make a follow-up appointment the next morning.
