You can use MyChart to go to TeleHealth visits, message your care team, keep track of appointments, view lab results, and pay your bill.

MyChart is an online tool, which we sometimes call a patient portal. MyChart helps you to keep track of your healthcare visits, test results, billing, prescriptions, and more. You can also send your provider questions through MyChart.

Who can sign up for MyChart?

Anyone who is a patient at an Equitas Health Medical Center.

What does MyChart do?

MyChart gives you 24-hour access to your health information online. You need a password touseMyChart. MyChart also uses an encrypted connection. Your password and the encrypted connection make sure that your personal information is safe from unauthorized access.

With MyChart, you can:

• Ask for appointments.
• See test results.
• Send messages to your doctor, nurse, dentist, or counselor.
• Go to video visits.
• Ask for medication refills.

How do I get started on MyChart?

Call your Equitas Health medical center. They will give you a special code to set up your account. After you enter the code, you will choose a user name and password.

Do I have to use MyChart?

Do what works for you! You can make appointments, refill your meds, and talk to our staff over the phone if MyChart doesn’t work for you.
