The Sliding Fee Discount lowers the cost of Equitas Health’s medical, dental, pharmacy, and counseling services for people at lower income levels.
If you do not have insurance, or your insurance is not enough to make your healthcare affordable, the Sliding Fee Discount may be a good option for you. To see if you can get the Sliding Fee Discount, please fill out an application before your appointment.

How much you might pay depends on federal poverty levels, your household earnings, and your household size. You will need to provide proof of yearly earnings for every member of your household in order to use the Sliding Fee Discount.

When you submit your application, please provide one of the following for yourself and each person in your household who is currently working:
For example: If you are paid every two weeks, provide (2) paystubs. If you are paid every week, provide (4) paystubs.

How do I prove other sources of income?
What if I am self-employed?
If you are self-employed, you will need to provide a detailed report of your most recent (3) months of income and business expenses. We must have as much information as possible about your income to see if you can get the Sliding Fee Discount.
What if I have no paperwork or documents that prove my income?
If you have no paperwork or documents that can prove your income, you might be able to submit a Self-Declaration of Income (DOI). A DOI is a signed statement that tells us why you are not able to provide another form of proof.
