How long will a medical records request take?
Equitas Health will provide access to the patient health information (PHI) requested in less than 30 business days from receiving your request. We realize there are circumstances when you may need your medical record more urgently. If this is your situation, please call the Medical Records Department to discuss your needs.
Is there are charge for medical records requests?
There is no fee for records sent to healthcare providers for continued care. For other requests, we will tell you in advance if there is a fee.
Use MyChart to attend TeleHealth visits, contact your care team, keep track of appointments, view lab results, and pay your bill.
